Important Information to Know

Eligibility for benefits is specific to your job class.
- Name changes: send to Human Resources at [email protected]
- Beneficiary information updates: complete a Change of Beneficiary Form and email it to HR at [email protected]
- Opt-Out Payment: The medical Opt-Out-Payment Form must be submitted within 30 days of enrollment to HR at [email protected]
- Qualifying Life Events: Documentation must be submitted to HR within 30 days of the Qualifying Event.
- If you are adding a NEW Dependent, supporting documentation must be emailed to the Benefits and Leave Manager at [email protected] within 30 days of completing your enrollment process:

- Marriage Certificate or Divorce Decree (Domestic Partners are ineligible)
- Birth certificates for children
- Social Security Number for all new dependents
- Court orders requiring coverage

- Your SSN or Employee ID Number
- Your PIN (last 4 digits of your Social Security Number and last 2 digits of your birth year-no spaces or dashes)
- IMPORTANT! If you do not wish to make changes to your current Trustmark or Wellfleet voluntary benefits, when you are enrolling, just click NEXT to go to the next benefit; do NOT make any selections. Your current benefits will carry over to the new plan year.

How to view and print your benefit statement-

- After logging in, click on the SIGN & SUBMIT button on the menu bar.
- Scroll to the BOTTOM of the page to open and print your confirmation statement.
Please note: you will only be able to view and print your benefit confirmation statement after your enrollment. You will not be able to make any changes in the system. If you need to make any changes, call our call center 484-370-4852 (Mon – Fri, 9am – 8pm ET)
