Important Information to Know


  • Your eligibility for benefits is specific to your job class.

  • Qualifying Life Events: Documentation must be submitted to HR within 30 days of the Qualifying Event.  
  • Name changes: send to Human Resources at [email protected] 
  • Beneficiary information updates: complete a Change of Beneficiary Form and email it to HR at [email protected]
  • Opt-Out Payment: The medical Opt-Out-Payment Form must be submitted within 30 days of enrollment to HR at [email protected]

  • If you are adding a NEW Dependent, supporting documentation must be emailed to the Benefits and Leave Manager at [email protected] after completing your enrollment process:
      • Examples of supporting dependent documentation-  
        • Marriage Certificate or Divorce Decree (Domestic Partners are ineligible).
        • Birth certificate for child(ren).
        • Social Security Number for all new dependents.
        • Court orders requiring coverage.
